New patients are always welcomed.
Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient in need of treatment.

Financial Arrangements
We are committed to providing quality services, and to the success of your treatment. Treatment options and findings will be discussed following your examination. We do not perform any procedures until we have informed and explained the fees to you.
We do offer our patients a variety of financial options. Patients who choose to pay for their services with cash or check will receive a 5% courtesy savings. Patient’s age 65 and older will receive a 10% courtesy savings.
We accept the following forms of payment: cash, check, Visa, MasterCard, Discover American Express, and CareCredit.

Care Credit
CareCredit is a separate line of credit that offers 3, 6,12, and 18 month interest free programs with low monthly payments. CareCredit also offers 24,36,48 and 60-month low interest payment plans.
CareCredit can be used to pay for co-payments and treatment not covered by insurance. With CareCredit you can start treatment immediately and leave your major credit cards available for unexpected expenses.

Dental Insurance Plans
Our office accepts most dental insurance plans. If your insurance is considered a PPO, please check in advance to verify whether or not Dr. Erlandson is part of your PPO network.
As a courtesy we will file your insurance claims for you, however on the date of service, all co-payments and deductibles are due.
